To speed up your registration process and to give you a more relaxing experience at our centre, we require you to complete this form before your first Chiropractic or Physiotherapy Visit.

INITIAL CONSULTATION

Your Initial Consultation will take between 45 & 60 minutes, whereby a detailed History & Examination will be performed by your Chiropractor or Physiotherapist. This information will then be analyzed and used to determine your Diagnosis, Prognosis and Treatment Plan.

SPINAL HEALTH CHECKS

Your Spinal Health Check will take approximately 30 minutes, whereby a Digital Posture Analysis and a Myovision sEMG Scan is performed. This information is analyzed and the outcome discussed with you together with any suitable advice.
