PAPERWORK

Please complete this admittance form so we have a full understanding of your past and current health situation.

COMPLETE HISTORY On your initial visit you will meet the doctor and discuss your health concerns. To better understand your personal health situation and to help improve treatment outcomes you will also be asked about your full medical history including previous injuries, illnesses, lifestyle, and family history.

EXAMINATION We will conduct a thorough examination to locate the cause of your problem and determine if you are a candidate for chiropractic care. The assessment will include range of motion, neurological and muscles testing, and an orthopedic assessment. The doctor may also need additional procedures, such as x-rays, to determine a diagnosis.

REPORT OF FINDINGS The doctor will explain the results of your examination. If we think that yours is a chiropractic case and that we can help, we will recommend a treatment plan and a schedule of care created just for you to promote your recovery and optimize your health.
