We are dedicated to providing exceptional lifestyle based Chiropractic Care for individuals of all ages.

We would like to review a fee simple commitments that will help you prepare for your first visit to our office.

Prepare for your First Visit

  1. Please complete all paperwork online at http://www.atwcenter.com and submit 48 hours prior to your appointment. This allows our Doctors to review your intake forms prior to your arrival. This is important for them to provide you with the best possible care.

  2. Please bring any prior medical or health records. If you would like to discuss your overall lifestyle please bring any lab work that you have had done within the past year. To better help our Doctors please bring any prior X-rays, CT scans, MRI scans, or other imaging that you may have had within the past year. Failure to do this step may delay any results or recommendations we can offer concerning your care.

  3. After scheduling an online appointment please contact our office to finalize any deposits needed to reserve your appointment time. Below is a statement about our Commitment to Appointments.

  4. What are your goals when it comes to your health and lifestyle? Our team is dedicated to helping you become healthier and reclaim your life. We know that the human body has more potential than most people realize. We are dedicate to helping you heal well, eat well, move well, think well, and live well. Please share your goals with us at your first visit!

Commitment to Care The Doctors and Team at Awaken to Wellness Center will listen and support you through your health journey. We know that trusting someone to join your health team is an important decision and we take that seriously. We will always make recommendations to help meet your needs and encourage you with research based information to improve you health and lifestyle.

We ask that you do your part to support your spine and nervous system. Healing and living a wellness lifestyle takes time. We use a team approach when providing care. By keeping appointments, and following our recommendations for home care we know you can live well.

Commitment to Appointments and Financial Considerations We will reserve time especially for you in our schedule. We will give your our complete attention and we will always do our best to limit any waiting time in our office.

Our office requires a DEPOSIT to hold your first visit appointment. This DEPOSIT is refundable if you cancel or reschedule your appointment within 24 hours of your appointment time. We understand that schedules change and we will do our best to accommodate your needs. Please call 502-454-3500 to make any appointment changes. If you scheduled through our ONINE BOOKING portal our team will be in contact to collect your Appointment Deposit. If we can not reach you to collect this deposit within 48 hours of scheduling an online appointment then your appointment time will be canceled.

Your insurance coverage is a relationship that you have established with your insurance company. As an out of network provider, we do not make care recommendations based on insurance coverage.

We make all health and wellness recommendations based on YOU and the results we want to achieve together. Our focus is YOU, not your insurance. Insurance plans are used to help cover crisis and sick care. For most patients this means they are symptomatic, and unable to perform their activities of daily living. They are typically seen in our office at a frequency of 2 to 3 x a week to help them regain the ability to function in their daily life. Insurance is used in the FIRST stage of care to help people heal, rehabilitate, improve their activities of daily living, and improve the health of the spine and nervous system. We can submit claims to your insurance using out of network benefits during the FIRST stage of care.

Insurance does NOT allow care to be used for wellness or to promote health. All services billed to an insurance company must be Medically Necessary.

"Medically Necessary Care" will be offered when functional improvement is expected to be achieved and can be measured, when you meet medical criteria associated with a health condition, and when care is expected to be short term with a specific goal to improve function. Functional Improvement is designed to help you in your daily activities with things like bending, turning, getting dressed, standing, and caring for yourself.

Insurance does NOT cover wellness visits and lifestyle visits meant to prevent health challenges of the spine or improve quality of life. In most offices, Wellness Care is typically provided every 2 weeks to every 3 months. These visits are NOT considered Medically Necessary, NOT covered by insurance, and if they are billed to insurance they are billed with a code that informs the insurance company that this visit was for wellness and is NOT medically necessary.

This is the law and not the choice of the doctor. If you have a (HSA) Health Savings Account or a (FSA) Flexible Spending Account, you can use the account to help cover your care in the office for medically necessary and wellness care as long as the care meets the guidelines outlined in your account rules and regulations. This allows you to use pre-taxed dollars for health care investments.

You have the right to rescind within 72 hours any obligation to pay for services performed in addition to listed fees.
