PAPERWORK

Please complete this simple intake form in advance so we have an understanding of your past and current health situation. Fill out as much as possible; if a question is not relevant to your situation, it's okay to leave blank. The form is best if filled out on your desktop, laptop or tablet. Please do not use your phone as the information will not display properly. If you are unable, you are welcome to fill out the form in office.

CONSULTATION Our doctors will sit down with you to discuss your health concerns and wellness goals.

EXAMINATION We believe in whole body healing starting with correcting and balancing your structure. Our thorough examination includes assessing your range of motion, muscle function, nervous system function, as well as a full body postural evaluation and orthopedic assessment. This will help us locate the root cause of your problem and determine if you are a candidate for chiropractic care. If you have previously taken x-rays or MRIs, please bring them with you to your first visit. In order to reduce your radiographic exposure, we do not x-ray in our office.

REPORT OF FINDINGS Immediately following your examination, the doctor will explain the results of your examination. If we think that we can help you, we will recommend a schedule of care created just for you. Our goal is to get you to your optimal structure and function as quickly as possible, allowing you to live your life fully! During this time we will also explain our very simple fee schedule and financial policy. Please note that we are out of network with all insurance companies, but will provide the appropriate paperwork to help you get reimbursed.

ADJUSTMENT If you are a candidate for chiropractic care in our office, we will conclude your initial visit with your first adjustment in our office.
